Summary

Repair the organization-owned review contract so required opencode-review / noema-review evidence fails closed unless a substantive Reviews API verdict is bound to the exact current head, while preserving review-only dispatch for Draft, stacked, and external/fork pull requests.

  • the conflicting Strix workflow and Caido bootstrap neutral-success regression were removed; typed provider non-pass remains owned by fix(strix): fail closed on incomplete provider scans #1153
  • branch updates were normal fast-forwards; no force-push or protection bypass was used

Evidence tied to predecessor heads is historical only.

Corrected control contract

  • Only a substantive exact-current-head APPROVED or CHANGES_REQUESTED review from the primary OpenCode reviewer can satisfy the formal-verdict requirement.
  • COMMENTED, dismissed, old-head, absent, placeholder, status/check-only, and deterministic fallback evidence remain non-passing.
  • Noema cannot accept its own predecessor verdict before current-head primary OpenCode approval.
  • Draft and stacked PR dispatch remains review-only and cannot mutate branch or merge state.
  • Targeted dispatch validates the target repository, live PR base, repository default branch, and exact head SHA separately.
  • External/fork heads are accepted only as untrusted review data. Coverage first fetches the exact head SHA and otherwise resolves refs/pull//head, binds it back to the dispatched SHA, and retries bounded propagation failures.
  • External heads remain excluded from scheduler branch-update, direct-merge, and auto-merge paths.
  • Scheduler environment isolation and trusted relative CodeGraph paths remain enforced.

Exact-head local verification

  • Full Python suite: 1,335 passed and 16 subtests passed.
  • Production statement coverage: 100% (8,238 statements); production branch coverage: 100% (3,210 branches).
  • Interrogate docstring coverage: 100% (760 items).
  • Passed: targeted actionlint for opencode-review.yml and pr-review-merge-scheduler.yml, compileall, Bash syntax, git diff --check, and Semgrep with 0 findings.
  • Local pip-audit is a real dependency failure on pip 26.1.2 (PYSEC-2026-3721, fixed in pip 26.2); that lock belongs to dependency-stack PR fix(security): repair pip audit and schedule orchestrator review #1198 and is not duplicated here.
  • The bounded local Strix run is not recorded as success; the oversized opencode-review-dispatch workflow hit the 15-second actionlint bound.

Hosted exact-head state

At the latest b096757 query: 4 completed-success, 13 completed-skipped, 1 in-progress, and 14 queued check runs; no completed failure was present. Fresh hosted workflows remain non-terminal. The predecessor 1c09f8d hosted Strix success is historical and does not substitute for b096757 evidence.

All recorded inline review threads are resolved, but the Reviews API has no qualifying exact-current-head formal verdict. Normal integration remains blocked.

Acceptance gate

Merge only through live protection after the unchanged exact head has terminal-success required checks, no valid unresolved findings/threads, and a qualifying independent approval. After protected-main integration, consumer canaries must show a real exact-head Reviews API verdict; a green check without that verdict remains a control failure.

@cwl-noema-review exact current head 8cec0a2c — required opencode-review/noema-review must fail closed when there is no current-head verdict. This is the orchestrator#176 root cause.

Hourly status (Grok, 2026-08-14T00:40Z) — Project #1 item set In Progress.

Buyer-felt root cause remains ContextualWisdomLab/contextual-orchestrator#176: green required opencode-review/noema-review with an empty Reviews tab. This PR (head 8cec0a2c) is the fail-closed increment. Local contract tests passed (62 passed). Reviewers stay edit: deny. LLM key stays NVIDIA_NIM_API_KEY. Two-approval policy stays.

Did not @opencode-agent (#954 is still open). Did not human-repository_dispatch as seonghobae. @cwl-noema-review already forwarded via github-actions[bot]; Noema on main will not post a verdict until a current-head OpenCode approval exists.

Deprioritized 22 queued leftover .github OpenCode dispatches in the #796#998 band so this PR's already-queued merge-scheduler scan-pr-queue can obtain a github-actions[bot] dispatch. In-progress reviews and other-repo product dispatches were left running.

After this lands on main: re-run required checks on starved PRs (including orchestrator#176) only after a github-actions[bot] dispatch posts a real Reviews-tab verdict.

Update 00:57Z: scan-pr-queue ran as github-actions[bot] and correctly waited (same-head Strix evidence is still running). Strix #1002 is in progress (31757559717). No OpenCode dispatch yet — that is correct until Strix finishes, then the scheduler workflow_run trigger must post the verdict. Still no Reviews-tab reviews. Did not @opencode-agent. Did not human-dispatch.

Update 01:37Z: github-actions[bot] dispatched OpenCode for head 8cec0a2c (31761122533) after Strix completed. Scheduler log: current head has completed Strix evidence; same-head OpenCode dispatched. Did not @opencode-agent. Did not human-dispatch.

Strix #1002 failed closed on NVIDIA NIM mid-stream Internal server error (nvidia_nim/nvidia/nemotron-3-super-120b-a12b). That is complete evidence for dispatch, but the required Strix check is still red until a later NIM-only rerun.

  • Head: a9880ae
  • Added the missing centralized workflow_run fixture to execute the centralized-dispatch branch in the scheduler filter.
  • Exact local tree: 1336 tests and 16 subtests passed; production statement/branch coverage 100%; interrogate 100%; compileall and diff check passed; Semgrep reported 0 findings.
  • Targeted actionlint passed for opencode-review, scheduler, security-scan, and Strix workflows. The 432 KB opencode-review-dispatch workflow did not finish in the bounded local actionlint run; hosted exact-head evidence remains required.
  • Hosted exact-head Checks are still in progress/queued, and no qualifying exact-head independent approval exists.

Decision: WAIT_AND_REMEDIATE; no merge or bypass.

  • pip-audit correctly remains a source dependency failure on pip 26.1.2 (PYSEC-2026-3721; fixed in pip 26.2), owned by dependency-stack PR fix(security): repair pip audit and schedule orchestrator review #1198. The lock was not duplicated into this PR.
  • Oversized opencode-review-dispatch actionlint exceeded the 15-second bound; local Strix was not promoted to success.

📝 Info: Required check trusts event head SHA without a live-head recheck

The inline jq gate checks reviews against github.event.pull_request.head.sha but never refetches the PR to confirm that SHA is still live, unlike the Python twin decide_required_verdict_check in opencode_dispatch_status.py, which fails closed on live_head != expected_head. Acceptable for pull_request_target since synchronize reruns the check, but the two implementations diverge on stale-head handling.
