feat: add managed Nginx deployments, monitoring, and upstreams

• add managed Nginx configuration deployments, live log viewing, and SQLite development tooling
• add proxy-host monitoring and managed upstream support
• build frontend assets inside the Docker image and add Docker Hub publishing workflow
• fix accessibility associations for upstream target form labels

Unfortunately, there are several points that prevent it from being reviewed in its current state:

• The CI is failing.
• The PR is very large (around 34,000 lines across 200 files) and provides little explanation.
• It combines several different objectives, such as managed configuration and publishing images to Docker Hub (under your own account)

Your help on the project is very welcome, but I’d recommend submitting smaller PRs, each focused on one specific change. This will make them much easier to understand, review, and discuss.
