docs: show human judgment capture and decision paths

• add accepted synthetic screenshots for the separate qualification-capture and live-run decision paths
• add exact Cloud merge commit and SHA-256 provenance for both assets
• keep the distinction explicit: qualification capture does not send a run; an answer does not prove an effect
