Preserve non-conflicting MCP entries from CODEX_CONFIG when building session config. Keep existing name-conflict filtering and replace same-name entries as a whole to avoid mixing transport schemas. Add regression coverage for env-only overrides, same-name precedence, session isolation, and non-object MCP config. Validation: typecheck and build passed; 559 tests passed, 26 skipped.
